With the awareness of this responsibility, we process your personal data within the framework specified below, as the Data Controller, within the scope of the Law on Protection of Personal Data No. 6698 (hereinafter, the Law) and the relevant legislation.

Within the scope of the law, in matters such as data processing, obtaining, recording, storing, updating, classifying, sharing with or transferring your personal data to third parties permitted by the legislation, by fully or partially automatic or non-automatic means provided that it is a part of any data recording system. It refers to any operation performed on the data.

COLLECTION, PROCESSING AND PROCESSING PURPOSE OF PERSONAL DATA

In order to provide you with high standards of service, your personal data, depending on the nature of the service provided, within the scope of the following purposes;

• To be able to present the products and services we offer by the company within the determined legal framework,

Within the scope of the personal data processing conditions and purposes specified in Articles 5 and 6 of the KVK Law, we obtain your personal data verbally, in writing, visually, or electronically, within the scope of the contract or from physical places and similar channels.

In this context, the main general and special quality personal data obtained within the framework of the activities carried out by our entire company and for the purpose mentioned above are listed below;

• Your identity data such as your name, surname, TR identity number, place and date of birth, marital status, gender information, and a copy of the TR Identity Card or Driver's License you have submitted,
• Your contact data such as your address, telephone number, e-mail address,
• Your financial data such as your bank account number, IBAN number,
• Certificates and documents required within the scope of health report and occupational safety
• Your answers and comments that you share with the aim of evaluating our services,

Your personal data listed above and your sensitive personal data may be processed within the scope of the personal data processing conditions specified in Articles 5 and 6 of the KVK Law and for the following purposes:

• Sharing the requested information with the Social Security Institution, Police and other public institutions and organizations in accordance with the relevant legislation,
• Fulfilling legal and regulatory requirements,
• Planning and managing the internal functioning of the institution by the Company Management,
• Providing training to our employees by Human Resources,
• Monitoring and preventing abuse and unauthorized transactions by the Audit and Information Systems departments,
• Invoicing for our services by the accounting department,
• Confirmation of your relationship with the institutions that have contracted with our company by accounting,
• Answering all your questions and complaints about our services by the Company Management,
• Taking all necessary technical and administrative measures within the scope of data security of our company's systems and applications by the Company Management and Information Systems departments,

Your "Personal and Private Data" mentioned above can be kept in physical and electronic archives within ARNES and external service providers with great care and compliance with the provisions of the legislation.

TRANSFERRING PERSONAL DATA

Your personal data shall be submitted to our business partners, suppliers, Company officials, shareholders, legally authorized public institutions and private persons, and to the following, within the framework of the Law on the Protection of Personal Data No. 6698 and other legislation and for the purposes described above.

• Ministry of Health, sub-units and family medicine centers affiliated to the ministry,
• Social Security Institution,
• General Directorate of Security and other law enforcement agencies,
• General Directorate of Population,
• Judicial authorities,
• Your authorized legal representatives,
• Third parties we consult, including lawyers, tax consultants and auditors we work with,
• Regulatory and supervisory institutions and official authorities,
• It can be shared with our suppliers, support service providers, archive service providers and business partners whose services we benefit from or cooperate with (for more detailed information, you can obtain information by applying to our company in writing.) within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the KVK Law. .

METHOD OF COLLECTING PERSONAL DATA AND LEGAL REASON

Your personal data is collected and processed in all kinds of verbal, written, visual or electronic media in order to carry out all kinds of work within the legal framework, within the scope of the above-mentioned purposes and ARNES field of activity, and in this context, for ARNES to fully and properly fulfill its contractual and legal obligations. . The legal reason for the collection of your data by these persons is the provisions of other legislation, especially the Law on the Protection of Personal Data No. 6698.

YOUR RIGHTS REGARDING THE PROTECTION OF PERSONAL DATA

In accordance with the law and relevant legislation;

• Learning whether personal data is processed or not,
• If personal data has been processed, requesting information about it,
• Accessing and requesting personal health data,
• Learning the purpose of processing personal data and whether they are used in accordance with its purpose,
• Knowing the third parties to whom personal data is transferred at home or abroad,
• Requesting correction of personal data in case of incomplete or incorrect processing,
• Requesting the deletion or destruction of personal data,
• Requesting notification of the third parties to whom personal data has been transferred, regarding the correction of personal data and/or the deletion or destruction of personal data in case of incomplete or incorrect processing
• Objecting to the emergence of a result against the person himself by analyzing the processed data exclusively through automated systems,
• You have the right to demand the compensation of the damage in case of loss due to the illegal processing of personal data.

DATA SECURITY AND RIGHT OF APPLICATION

Your personal data is meticulously protected within technical and administrative possibilities, and necessary security measures are provided at a level appropriate to possible risks, taking into account technological possibilities. In accordance with paragraph 1 of Article 13 of the KVK Law, you can send your request to our company to exercise your above-mentioned rights in writing or by other methods determined by the Personal Data Protection Board.

By requesting and filling out the "Application Form pursuant to the Law on the Protection of Personal Data" from our company for your requests under the Law, and your request containing the necessary information to identify you in order to exercise your above-mentioned rights, and your explanations regarding your right to exercise from the rights specified in Article 11 of the KVK Law;

Our company will conclude the request free of charge as soon as possible and within thirty (30) days at the latest, depending on the nature of the request. However, if the transaction requires an additional cost, the fee determined by the Personal Data Protection Board will be charged by us.
